Registered Nurse (RN), Case Manager

What you'll do

• Conduct admissions, resumption-of-care and follow-up assessments using OASIS
• develop and evaluate plans of care
• administer medications and treatments
• teach clients and caregivers
• coordinate interdisciplinary care
• document clinical records
• support discharge planning
• supervise LPNs and home health aides
• make supervisory home visits at least every 14 days
• participate in backup call as needed.
