The physician supports Asian Health Services’ SAGE clinic and its care for older adults. The physician provides primary and consultative care for geriatric patients and leads an interdisciplinary team to address complex medical and social needs. The role advances comprehensive, culturally competent care and supports improved quality of life for senior patients.

What you'll do

• Provide primary and consultative medical care to geriatric patients
• assess, diagnose, treat, and manage acute and chronic conditions
• lead the SAGE interdisciplinary team
• conduct screenings and health assessments
• collaborate with specialists and care team members
• advocate for seniors and help navigate community resources
• document care in EHR systems
• mentor clinical staff and trainees
• participate in quality improvement and geriatric care protocol development
• serve as a geriatric liaison to the PACE program.
