Physical Therapist – Mobile Outpatient Therapy

Provide outpatient rehabilitation services in an in-home setting for the older adult population. Evaluate and treat patients in their private residence or independent living facility. Assess the need for, develop, and deliver physical therapy programs. Identify and make referrals to other disciplines.
