What you'll do

• Provide nutrition assessments for admissions, quarterly and annual reviews, tube feedings, and significant health changes.
• Conduct nutrition education for residents, update care plans, complete MDS documentation, and perform nutrition-focused physical exams.
• Participate in care conferences, clinical meetings, and QAPI programs while conducting monthly kitchen inspections.
