What you'll do

• Conduct Start of Care admissions using OASIS
• complete follow-up and evaluation visits as needed in the designated territory
• visit patients in their homes
• perform assigned medication reconciliation, wound care, treatments, and procedures
• monitor client conditions and report changes to the clinical team
• collaborate with physicians and agency staff
• document observations, interventions, evaluations, and services
• initiate and administer IV medication infusions according to established protocols.
