The nurse provides one-on-one skilled care for a five-year-old patient with a G-tube, tracheostomy, and ventilator. The role supports the child’s care plan and helps the child remain at home and in school with family-centered support.

What you'll do

• Provide skilled nursing care in home and school settings
• assess and monitor the patient and communicate changes
• administer medications, treatments, and prescribed therapies
• provide tracheostomy, ventilator, airway, and G-tube care as needed
• document care and educate the family
• follow individualized care plans.
