What you'll do

• Conduct nutritional assessments
• identify residents at nutritional risk and recommend interventions
• develop and revise individualized nutrition plans
• monitor weight and intervention outcomes
• recommend therapeutic diets, supplements, hydration programs, and enteral nutrition as appropriate
• document assessments and follow-ups
• participate in care plan meetings
• review menus
• provide nutrition education and counseling
• support nutrition-related quality improvement and survey preparation.
