What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine admission and recertification eligibility
• develop, implement, and document plans of care
• educate patients and families on self-care, nutrition, and dietary needs
• determine service scope and frequency
• perform preventative and rehabilitative nursing procedures
• apply infection-control precautions
• provide clinical direction to home health aides and LPN/LVNs
• prepare clinical and progress notes
• communicate changes and outcomes to physicians and other personnel
• monitor cases for third-party-payer compliance.
