What you'll do

• Provide peer recovery support within training, certification, and organizational policy
• assist clients with recovery planning tools and care transitions
• conduct assigned housing checks
• encourage relapse prevention and outreach after relapse
• connect clients with transportation, housing, food, clothing, meetings, and community resources
• maintain supportive alumni contact
• transport clients as applicable by site.
