What you'll do

• Direct the home care department and ensure workflows and policies are followed
• hire, train, and coach home care staff
• assess the effectiveness of nursing assessments and interdisciplinary team participation
• ensure nurses deliver and document interventions in participant care plans
• oversee care quality in nursing homes and board-and-care facilities
• arrange and supervise nursing, physical therapy, and occupational therapy.
