The Medical Home Direct Support Professional role is onsite in Concord, NH, with supported homes throughout the listed New Hampshire communities. Pay is $18–$20 per hour. Community Bridges provides residential and day services to adults with intellectual and developmental disabilities. The role helps clients build independence and participate in daily life by teaching living skills, supporting personal care and health needs, and providing companionship. It also supports clients’ community participation through outings and transportation to appointments and activities.

What you'll do

• Provide companionship and engage clients in activities
• assist with bathing, dressing, grooming, cooking, cleaning, and eating
• transport clients to appointments and activities
• administer medications
• observe, document, and report daily activity data. Overnight duties, when applicable, include personal care and bedtime routines, light housekeeping, bed checks, documentation, and morning routines.
