Pharmacist — Home Infusion is an onsite position in West Palm Beach, FL, paying $58–$65 per hour. InfuCare Rx provides home infusion and specialty pharmacy services for patients receiving IV therapy in the home. The pharmacist supports safe and effective home infusion therapy through clinical assessment, medication review, monitoring, consultation, and coordination with patients and care teams. The role also supports sterile preparation, dispensing, documentation, and pharmacy quality activities.

What you'll do

• Coordinate new referrals and admissions
• review and verify patient orders
• assess patient, drug, and disease information
• support therapeutic objectives and monitoring
• evaluate laboratory and clinical data
• communicate monitoring results
• program infusion devices as indicated
• counsel patients and care teams
• document clinical activities
• prepare progress reports
• supervise or participate in sterile admixtures and dispensing
• maintain pharmacy records
• and participate in training and quality activities. On-call rotation applies if applicable.
