What you'll do

• Orient new patients to program rules, policies, procedures, and rights
• conduct intakes, assessments, discharge planning, and treatment planning

• use screening tools and escalate high-risk concerns to clinical and medical leadership
• coordinate referrals and community resources
• document counseling services
• attend training and team meetings.
