Hospice Registered Nurse (RN) Case Manager

The case manager assesses patients’ needs, develops and documents care plans, and coordinates services with families, physicians, and the interdisciplinary team. Through clinical guidance, education, and ongoing evaluation, the nurse supports continuity and quality of end-of-life care.

What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine hospice admission and recertification eligibility
• develop, implement, and document care plans
• educate patients and families about disease processes, self-care, end-of-life care, and nutrition
• determine service scope and frequency
• provide nursing procedures and infection-control practices
• direct hospice aides and LPN/LVNs
• document progress
• communicate changes and outcomes to physicians and care personnel
• monitor cases for payer compliance
• perform assigned on-call responsibilities.
