What you'll do

• Oversee clinical day-to-day operations, including quality initiatives, staffing, in-service education, onboarding, and clinical service delivery.
• Assess referred patients’ home health care needs, coordinate intake and care planning, collaborate with physicians, nurses, and discharge planners, ensure plans of treatment and needed services are arranged, schedule field staff visits, obtain authorizations when appropriate, and conduct case conferences.
• The role requires a New York RN, PT, or OT license, a BSN, and at least three years of pediatric OPWDD management-level experience in a CHHA.
