Care Navigator — HIV Care and Treatment Adherence

What you'll do

• Engage patients with adherence needs and those who have fallen out of care
• coordinate referrals with medical and behavioral health providers
• guide and advocate for patients accessing internal and community services
• maintain regular patient contact and individualized care plans
• identify and address barriers to retention
• follow up after hospitalizations, emergency visits, missed appointments, or medication refills
• coordinate with clinicians and community organizations
• maintain accurate electronic records and reports
• participate in quality improvement and ongoing training.
