What you'll do

• Manage a home health caseload
• perform skilled nursing visits, assessments, and care-plan updates
• complete admissions, recertifications, resumptions of care, and discharges
• complete OASIS assessments and documentation
• coordinate with physicians, therapists, caregivers, and interdisciplinary team members
• educate patients and families
• monitor health changes
• administer ordered medications and treatments
• maintain required compliance documentation.
