Job overview
The Director, Inpatient System Care Management is an onsite role based in Oakland, CA. This position supports the SYS Care Coordination department at Highland General Hospital, part of the Alameda Health System. This role exists to manage the day-to-day operations of inpatient Care Management across all AHS facilities. The Director ensures efficient patient flow from admission through a safe and clinically appropriate discharge.
