Registered Nurse (RN) MDS Coordinator (RNAC)

The Registered Nurse (RN) MDS Coordinator (RNAC) role is onsite in Bethlehem, Pennsylvania, with a Monday–Friday day schedule from 7:00 a.m. to 3:00 p.m. Pay is up to $41 per hour. Holy Family Senior Living provides a continuum of services for older adults and their families. The coordinator manages resident assessment and documentation processes, develops individualized care plans, and works with the interdisciplinary team to support resident outcomes. The role also monitors regulatory compliance and accurate coding used in reimbursement and case-mix determination.

What you'll do

• Complete and coordinate RAI/MDS assessments and related documentation within required timeframes
• develop individualized resident care plans
• coordinate with the interdisciplinary team and lead assigned care-plan or MDS meetings
• audit clinical documentation for accuracy, timeliness, compliance, and coding
• support accurate reimbursement and case-mix determination
• maintain knowledge of MDS/RAI requirements and prepare resident status updates.
