Physical Therapist, Developmental Disabilities

Job overview

The Physical Therapist role provides in-home and community-based services across New Jersey, with a statewide service area and flexible self-directed scheduling. Compensation is $76–$82 per hour. Beyond Home Health Care provides services to people with intellectual and developmental disabilities through New Jersey DDD programs. The therapist evaluates functional movement and delivers individualized interventions to support physical independence, prevent secondary complications, and promote community participation in line with participant goals and DDD guidelines.

What you'll do

• Evaluate mobility, strength, balance, gait, and functional movement
• develop individualized treatment plans aligned with each participant’s Individualized Service Plan
• provide one-on-one therapy in homes and community settings
• document sessions, progress, and outcomes
• collaborate with support coordinators, families, caregivers, and the interdisciplinary team
• provide caregiver training and home programs
• follow DDD, incident-reporting, and HIPAA requirements
• maintain accurate billing records.
