What you'll do

• Review referrals and patient information to determine home care needs and assign clinicians
• evaluate services and guide clinical staff
• review diagnoses, medications, procedures, and clinical course
• help establish goals and plans of care
• attend case conferences
• conduct quarterly record reviews
• assist with hiring, orientation, training, and policy development
• participate in rotating on-call coverage, including one weekend per month.
