Case Manager

The Case Manager role is onsite in Longmont, CO, with a Monday–Friday schedule from 5:00 a.m. to 1:30 p.m. Compensation is $22.00–$26.00 per hour, plus a bonus structure. Behavioral Health Group provides opioid addiction treatment services and describes its mission as helping patients find recovery. The Case Manager coordinates individualized substance use disorder care, connects patients with internal and community resources, and supports continuity of treatment. The role also maintains care plans and records and collaborates with the treatment team to address patients’ recovery needs.

What you'll do

• Provide substance use disorder case management
• orient patients to program services, policies, and expectations
• assess social, educational, vocational, and recovery needs
• make referrals and coordinate follow-up
• provide crisis intervention and support
• develop and maintain treatment plans and records
• assist with program implementation, process improvement, and caseload management
• participate in community outreach and engagement.
