Job overview

The Case Manager II role is onsite in Saint Paul, MN, with pay of $25–$28 per hour. The position is assigned to Higher Ground St Paul and may be assigned to program sites with the greatest operational need. Catholic Charities Twin Cities serves people in need and advocates for justice in the community. The case manager provides direct support to adults with serious mental illness and co-occurring conditions who are experiencing homelessness or housing instability. Through assessment, individualized service planning, advocacy, and referrals, the role helps clients access resources and build long-term stability, safety, and independence.

What you'll do

• Assess client needs and prepare, evaluate, and maintain individualized service plans
• help clients obtain services, benefits, and supports, including documentation and applications
• coordinate referrals and community-provider partnerships
• maintain accurate case records
• provide education and resource information
• collaborate with supervisors and team members on case planning
• engage community systems for linkage, crisis management, and follow-up
• provide services to a complex caseload, which may include in-home visits
• may transport clients when appropriate.

Why this role
