What you'll do

• Develop and maintain relationships with hospitals, health systems, physicians, case managers, and community referral sources
• support skilled nursing census growth and market development
• mentor and coach Patient Care Liaisons through ride-alongs and field visits
• coordinate with clinical leadership and facility administrators on hospital discharge communication
• report market insights and use CRM systems to track outreach, referral sources, and strategic accounts.
