What you'll do

• Arrange aftercare placements and referrals, including sober living referrals
• monitor program completion and discharge dates
• coordinate discharge planning with relevant departments
• maintain relationships with referral sources
• communicate with clients’ families
• organize visits and verify clients return safely
• share discharge updates and maintain transition communications and KIPU documentation
• distribute facility mail.
