The role exists to build community and healthcare referral relationships, engage prospective participants and caregivers, and support program growth through qualified referrals and enrollments. It works with enrollment staff and the interdisciplinary team to help participants move from initial referral through enrollment.

What you'll do

• Develop and execute outreach plans aligned with growth goals
• identify and cultivate referrals from healthcare providers, community organizations, and senior service networks
• conduct consultative conversations with prospects and caregivers
• document outreach and prospect activity in the CRM
• maintain referral partner relationships
• represent BoldAge PACE at community events and educational presentations
• use demographic, market, and referral information to guide outreach
• collaborate with enrollment staff, benefits specialists, assessors, and interdisciplinary team members
• work toward referral, enrollment, and outreach goals.

Why this role

High school diploma required; three years of community outreach, relationship-based sales, or business development experience required. One year working with frail and elderly people is required, or training will be provided if absent.
