Care Coordinator — OhioRISE

Job overview

The Care Coordinator — OhioRISE role is based in Athens, OH, and provides services in families’ homes, communities, or preferred locations. The pay range is $20.19–$25.03 per hour, based on experience, education, and/or licensure. Integrated Services for Behavioral Health serves people across Southeastern and Central Ohio through behavioral health and related services; OhioRISE supports youth ages 0–20 with complex behavioral health and/or substance use needs.

What you'll do

• Create, plan, and organize a family-defined care team
• lead and facilitate child and family team meetings
• partner with youth, families, natural supports, and service providers to identify needs and services
• support youth at home, school, and in the community, with the goal of keeping families together and reducing hospitalizations
• maintain documentation for OhioRISE benefits and use databases and electronic health record systems. The role requires travel within the assigned service area and attendance at mandatory in-person and virtual trainings and supervision.
