What you'll do

• Develop relationships with healthcare providers and community organizations

• coordinate referrals, admissions, and transitions
• communicate clinical information confidentially
• conduct facility visits, presentations, and outreach
• monitor referral activity and admissions
• support discharge planning
• maintain documentation and regulatory compliance.
