Clinical Care Coordinator, Utilization Management

Job overview

The Clinical Care Coordinator role is onsite in Fresno, CA; pay is $30–$36 per hour. The position supports Sante Health’s Utilization Management department, which coordinates care among physicians, health plans, hospitals, and other providers. The coordinator manages care transitions and follow-up for members after inpatient or emergency department care and for people with complex medical conditions. The role supports improved health outcomes and aims to reduce emergency department use and inpatient readmissions.

• Conduct initial and transitional assessments
