The Home Health Aide / Certified Nursing Assistant role is onsite in Crete, IL, serving home health patients in Will County. Pay is $25–$35 per visit. Safe Life Home Health Care provides compassionate home health care and support to patients in the community. The aide assists patients with daily living, mobility, hygiene, meals, and medication reminders; monitors and records vital signs and care in the electronic medical record; and reports changes to the nursing team. The role also supports patient safety, communication, transportation, and coordination with nurses and therapists.

What you'll do

• Assist with bathing, dressing, grooming, toileting, mobility, and transfers
• take and record vital signs
• provide medication reminders
• help with meal preparation and feeding
• maintain hygiene and comfort
• observe and report changes
• document care in Kinnser by WellSky
• communicate with patients, families, and the care team
• follow infection-control and safety protocols
• provide transportation as needed
• offer companionship and emotional support
• collaborate with nurses and therapists
• educate patients and families on self-care and safety.
