Hospice Registered Nurse (RN) – Night On-Call is an onsite role based in Webster, NY, with a stated salary range of $70,824–$99,862.80 per year. UR Medicine Home Care’s hospice team provides care and support to patients and families at home. The schedule is 4:30 PM–8:00 AM, alternating between three and four days per week; this is an on-call position, with home visits when patient needs arise. The nurse responds to patient-related calls, resolves concerns when possible, and provides home visits for assessment, treatment, and support. Care follows the plan of care and includes attention to patients’ and families’ physical, psychological, social, and spiritual needs.

What you'll do

• Provide nursing care to patients, families, and caregivers in compliance with applicable requirements
• receive patient-related calls and resolve issues when possible
• prioritize and make care decisions
• make home visits as needed for patient information, treatment, and support
• provide services according to the plan of care and identify additional needs
• visits may include pronouncement of death and crisis management in consultation with hospice leadership.
