Case Manager, Homeless Shelter Services

Job overview

The Case Manager role is onsite in Omaha, NE, primarily Monday–Friday, usually 8:00 a.m.–4:30 p.m., with some later hours possible. Compensation is not specified. Siena Francis House provides shelter and support services to people experiencing homelessness. The case manager assesses client needs, develops goal plans, and coordinates referrals and wraparound services. By supporting stabilization and progress toward stable housing, the role contributes to the shelter’s services for homeless individuals.

What you'll do

• Screen and assess clients
• develop and review goal plans
• maintain scheduled client contact
• provide intensive case management and direct assistance
• connect clients with medical, housing, financial, and community services
• maintain records and document interactions
• complete annual recertifications and inspections
• coordinate with community agencies and follow applicable data workflows.
