Behavioral Health Care Coordinator

Job overview

The Behavioral Health Care Coordinator role is onsite in Live Oak, FL, with stated pay of $16–$18 per hour. The position supports the Managed Care department at North Florida Pediatrics, helping rural patients navigate behavioral health and substance-use treatment. The coordinator connects patients, primary care providers, behavioral health services, and community supports. Through care plans, check-ins, referrals, documentation, and crisis follow-up, the role helps patients access coordinated care and supports continuity between primary and behavioral health services.

What you'll do

• Develop personalized care plans
• coordinate monthly telephonic or telehealth check-ins and chronic care management
• help patients locate clinics, mobile outreach, and regional behavioral health centers
• connect primary care providers with psychiatric consultants, emergency services, and community support teams
• guide patients through virtual consultations and therapy
• maintain patient tracking and compliance documentation
• follow up after emergency-room visits or crisis interventions.
