What you'll do

• Build and maintain relationships with OPWDD care managers, CCOs, schools, hospitals, clinics, social workers, and community organizations
• explain available home care, nursing, therapy, and support services
• attend outreach and networking events
• assist families navigating OPWDD eligibility and service coordination
• coordinate referrals with intake, eligibility, and clinical teams
• track referrals and pipeline activity in CRM systems
• develop partnerships across New York City.
