Clinical Care Leader (CCL) - Full Time

Utilizes the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the home setting. Collaborates with resident, family, and inter-disciplinary colleagues. Documents resident history, symptoms, medications, and care provided. Utilizes technology to enhance care and collaborate with team members.
