What you'll do

• Assess residents’ needs and develop and update individualized service plans with clinical staff, residents, and families
• guide and train caregiving staff and support scheduling

• ensure compliance with regulations and company policies
• conduct audits and participate in quality assurance
• and coordinate services with healthcare providers and community leadership.
