What you'll do

• Conduct assessments and develop person-centered care plans
• coordinate services across treatment teams and primary care providers
• facilitate follow-up for hospitalizations and life transitions
• monitor client progress and HEDIS gaps
• provide health education and crisis response
• maintain accurate documentation
• support clinic operations and collaborate with Nurse Practitioners
• participate in marketing and outreach initiatives.
