What you'll do

• Meet with families to understand priorities and develop personalized plans
• provide dementia education and caregiver coaching
• help patients remain at home and in their community when safe
• connect families with community resources and follow through
• check in by phone, video, or in person
• respond to crises and transitions
• collaborate with interdisciplinary teams and community partners
• facilitate education classes and support groups
• document services in the electronic health record.
