Medical Director / Medical Director Designee — Hospice

The Medical Director / Medical Director Designee — Hospice role is onsite in San Jose, CA, with compensation of $160–$200 per hour. Heart of Hope Asian American Hospice Care is a nonprofit hospice serving patients and families facing end-of-life challenges in the Silicon Valley area. The physician provides clinical and medical direction for hospice services, supports regulatory compliance and quality of care, and guides the interdisciplinary team. The role also provides direct physician services to hospice patients and helps coordinate individualized, patient-centered care.

What you'll do

• Provide clinical supervision and administrative support
• monitor care quality and regulatory compliance
• lead and participate in interdisciplinary team conferences and care planning
• support admissions and hospice eligibility certifications
• consult with attending physicians and staff
• help develop individualized plans of care
• arrange 24-hour on-call physician availability
• provide assigned inpatient and home-care physician services
• maintain required records.

Why this role

Requires an unrestricted California Physician and Surgeon license and at least two years of recent supervisory or managerial experience in hospice, home health, or palliative care.
