Home Health Registered Nurse (RN) – One-on-One Patient Care

The Home Health Registered Nurse (RN) role is onsite, providing care throughout Mesa County, CO; the listing location is Grand Junction, CO. Compensation is $35–$43 per hour plus mileage reimbursement. SummitWest Care is a nonprofit home health agency focused on ethical, patient-centered care in the home. The RN provides skilled one-on-one nursing care, develops and updates individualized care plans, monitors patient progress, and coordinates with patients, families, and an interdisciplinary team. The role supports patients in remaining safe, independent, and comfortable at home and includes participation in an on-call rotation.

What you'll do

• Perform comprehensive patient assessments
• develop and update individualized plans of care
• provide skilled nursing care and interventions
• monitor progress and communicate changes to the care team
• educate patients and families about medications, treatments, and care plans
• coordinate with LPNs, therapists, caregivers, and providers
• complete timely EMR documentation
• participate in the on-call rotation.
