What you'll do

• Promote home health and hospice services in Las Cruces and surrounding communities
• build referral partnerships with providers, skilled nursing facilities, clinics, and local agencies
• educate patients, families, and professionals about Medicare hospice benefits and eligibility
• coordinate referrals and follow up
• help develop territory marketing strategies
• represent Sierra at community events and in-service programs
• analyze referral trends
• and collaborate with clinical teams.
