What you'll do

• Carry out physician-prescribed care plans
• document care
• administer prescribed medication, treatment, and therapies
• assess patients and coordinate care
• monitor vital signs and intake and output
• assess skin and provide wound care
• monitor condition changes
• communicate with family, physicians, and case managers
• educate patients, families, or home health aides
• collaborate with and supervise nursing assistants and personal care attendants.
