What you'll do

• Maintain confidential client relationships and files
• conduct intake and psychosocial assessments
• develop and revise independent living plans
• connect clients with resources and referrals
• assist with applications
• monitor and document progress
• coordinate with referral organizations
• provide required reports. Some follow-up and other tasks may be assigned as described.
