The Dietitian - PRN is an onsite role based in Baltimore, MD. Compensation is not specified in the posting. This position supports the Food Rx program, a national award-winning initiative at MedStar Health that provides nutrition counseling and medically tailored groceries to community members with chronic diseases and food insecurity. The role exists to improve health outcomes for patients by providing diabetes education, weight management strategies, and meal planning. The Dietitian works within an interdisciplinary team to identify eligible patients, coordinate enrollment, and incorporate evidence-based strategies into population health management.

What you'll do

• Work closely with Community Health Advocates across 4 Baltimore region hospitals to address social needs of Food Rx patients.
• Collaborate with providers in the Collaborative Care Program outpatient clinic to identify eligible patients, coordinate enrollment and follow-up, and provide MNT/DSME consults.
• Provide diabetes and cardiovascular education, nutrition counseling, weight management strategies, and meal planning.
• Report to the Regional Clinical Manager for Food Rx.
• Incorporate evidence-based strategies into population health management for patients with complex medical and social needs.
