What you'll do

• Manage an assigned home health patient caseload from admission through discharge
• coordinate care plans with physicians, therapists, field clinicians, patients, caregivers, and interdisciplinary teams
• review and approve clinician documentation
• monitor patient progress and outcomes
• communicate condition changes to providers
• ensure documentation meets agency, Medicare, and regulatory requirements
• maintain records in WellSky (Kinnser)
• participate in case conferences and care planning
• provide clinical guidance to field staff
• support compliance, recertifications, discharge planning, and quality improvement.

Why this role
