Community Health Worker (CHW) is an onsite role based in Carson City, NV, with work in homes, community settings, or recovery settings. Compensation is $23 per hour. NovumHealth CHWs help people navigate mental health, substance use, and social barriers by building trust and connecting them with care and resources. The role exists to assess community health needs, identify gaps in care, and help individuals access services. Through care coordination, outreach, and appointment support, the CHW contributes to comprehensive care plans and improved access to community resources.

What you'll do

• Assess individuals’ healthcare needs and identify gaps in care
• collaborate with healthcare providers, social service agencies, and community organizations on care plans
• help patients navigate healthcare services, appointments, referrals, diagnostics, and community resources
• conduct home visits or outreach to assess living conditions, support systems, and treatment-plan adherence
• provide transportation to appointments in a personal vehicle when needed, with mileage reimbursed.

Why this role

Requires active, valid Community Health Worker certification and at least one year of related experience. CPR/First Aid certification is required or must be obtained.
