What you'll do

• Communicate discharge plans
• coordinate discharge dates with the interdisciplinary team
• complete discharge paperwork and documentation
• coordinate durable medical equipment, referrals, and transportation
• provide education
• participate in quality indicator review and corrective actions
• analyze quality assurance reports and present to the Quality Assessment and Assurance Committee
• attend interdisciplinary meetings
• assist on the floor as needed
• identify safety hazards and educate staff.
