What you'll do

• Help individuals and families access medical, social, educational, and other support services
• provide counseling and therapeutic services as specified
• assess, advocate, link, and coordinate community-based services
• support client goals
• meet regularly with clients and families at home or other community locations as needed. Requires crisis-prevention and universal-precautions training, a valid driver’s license, automobile insurance, and reliable transportation.
