The role supports individual service and behavior support plans, maintains daily records, and helps foster positive relationships with each person’s support network.

What you'll do

• Assist with hygiene, grooming, feeding, ambulation, medical monitoring, and health-related tasks
• encourage self-help activities
• implement Individual Service Plans and Behavior Support Plans
• prevent or de-escalate aggression
• document activities, medications, drills, goals, and data
• transport individuals to community activities and medical appointments.
