What you'll do

• Conduct diagnostic assessments and develop individualized treatment plans
• provide individual, group, and family counseling
• document sessions to Medicare and facility requirements
• participate in care planning and interdisciplinary discussions
• support residents and families
• coordinate with psychiatric nurse practitioners and facility staff
• educate facility staff on psychosocial needs and referrals.
